Required Forms

These forms must be completed and retained at the district and/or school level. A copy of these forms will be required by the OSDE for districts and/or schools that are being monitored.

Qualtrics Test Security Forms

Qualtrics is the new test security software. Each person who holds one of the five roles during the test administration (District Superintendent, District Test Coordinator, Building Principal, Building Test Coordinator, Test Administrator) must fill out their individual form when the test administration is completed.
